Nociceptive and neuropathic pain

A 65 years old male patient presented to pain clinic with complaint of chronic lower back pain for the past 6 months, associated with numbness and tingling sensation in right lower limb. Pain was constant and sharp in nature and associated with burning sensation, progressive and increased on movement. MRI lower back revealed PIVD L4-L5 and L5-S1 associated with right lower limb radiculopathy. Patient is a known diabetic for the past 15 years and on treatment with oral hypoglycaemic agents with good control. The patient has taking various medications including NSAIDs/ multiple course of steroids/ opioids and physiotherapy with no relief.
